BEST OPTION - The best form of ID to bring is a valid U.S. Passport and to know your social security number. This is both for an adult or minor. NOTE- WE WILL NEED TO TAKE A PHOTO OF YOUR PASSPORT OR ANY OTHER FORMS OF ID IN LIST A.

OTHER OPTIONS

Over age 16 | Adults

Any one form of the following:
1. Drivers license
2. ID card issued by a State
3. School Id card with a photograph
4. Voter's registration card
5. U.S. Military card or draft record

AND one of the following:
1. Original social security card
2. Original or certified copy of birth certificate

*Total of two forms of ID

Under age 18 | Minors | Infants

Any one form of the following:
1. Photo school ID
2. School report card
3. Immunization record/card
4. Clinic, doctor, or hospital record
5. Day-care or nursery school record

AND one of the following:
1. Original social security card
2. Original or certified copy of birth certificate

*Total of two forms of ID

We understand these are very important documents and we do not want you to lose them, but production is required by law to review these documents in order for you to work with us.
